Application Process

Student applications for the
2008-2009 school year will be available on Sunday, October 7,
2007, in the lobby of Stephens Hall from 8:30am to 1:30pm.

Completed applications for the
2008‑2009 school year should be filed by February 22, 2008.

All applications must be returned in
the envelope provided by first-class mail to St. Mary's School via
the U.S. Postal Service and postmarked no earlier than October 8,
2007. Each envelope will be date and
time-stamped upon receipt. St. Mary's does not accept applications
that are hand-delivered; or that are returned by certified mail,
registered mail, Federal Express, second-day express mail, express
mail, e-mail, fax; or in any other envelope. Only one application
per child will be accepted; any other applications and fees received
will be discarded.

Before an application can be
considered:

Please print and leave no blank
spaces. For questions that do not apply, write “N/A” on the
line.

Please sign and date your
application.

Enclose a non-refundable
application fee of $40.00 per applicant made payable to St.
Mary’s School.

If Catholic, include a copy of
baptismal (required), and reconciliation and Eucharist
certificates (if applicable).
-
If non-Catholic, include a copy
of the birth certificate.
-
Include current plus two previous
years’ report cards.
-
Include current plus two previous
years’ standardized test scores.
-
Include a copy of a custody
decree (if applicable).
-
Immunization records, the School
Entrance Health form, and the Student Information Form may come
under separate cover, but must be received before a child is
accepted.

Please give the Student Information
Form to your child’s current teacher, who is responsible for
returning it to St. Mary’s.

For first through eighth grades, the
Release of Student Records Request Form authorizes the child’s
current school to send copies of progress reports/grades and
standardized test scores to St. Mary’s.

Available openings in all grades are
offered in the order that follows. Applications will be placed in
the appropriate category by the time and date on which they were
received:

Siblings of returning students
currently attending our kindergarten through eighth grade who
are children of
active parishioners of St. Mary’s Catholic
Church

Siblings of returning students
currently attending our kindergarten through eighth grade who
are children of non-parishioners

Children of
active parishioners

at parishes without schools

Please
note: Attendance in St. Mary’s pre-kindergarten program does not
guarantee a space in kindergarten.
Catholic Church parishioner status is
confirmed with the applicant’s home parish office. To qualify for
St. Mary's in-parish tuition rate, formal registration and active
participation in parish life, as measured by regular offertory
envelope use, for a period of one year is required.
